BAND QUARTERLY PROGRESS REPORT

The following is a quarterly report of your child's progress in Beginning Band. Each item is graded on the following scale:

1 = Poor
2 = Fair
3 = Good
4 = Excellent
n/a = not applicable at this time

All of the items are then averaged for your child's quarterly progress grade in band.

Please sign at the bottom of the form to show that you have seen this report. Also, please write any comments or questions you may have and return this to me as soon as possible.
