SUBJECT: Required Reports for Annual Updates for Deaf-Blind & Legally Blind Students


Enclosed is information and instructions for completing two separate reports which are required by federal regulations. One is the Deaf-Blind Census Report that must be submitted by all programs serving children and youth with deaf-blindness. The second is the Federal Quota Registration of Legally Blind Students.

Specific instructions and forms are included with this memorandum. Refer to Attachment A for the Deaf-Blind Census and Attachment B for the Legally Blind Registration. Remember, these are two separate and distinct items which must be addressed and submitted to the Exceptional Children Division by Monday, February 23, 2009.
