FORMS

The State Forms are used by Exceptional Children Personnel within the traditional LEAs, charter schools and state operated programs to assist with the information gathering procedures/process for children with disabilities. INSTRUCTIONS FOR NEW FORMS

There are now two tables of forms. The new 2008 directions and forms are in English and in MS Office Word format for ease of use. The Spanish forms will be posted as soon as possible. As each new set of directions and forms are posted, the previous ones will be removed.

The font for the directions has been decreased from the size that was used in the training documents to reduce the number of pages when printed, not the importance of the directions.

INSTRUCTIONS FOR PREVIOUS FORMS

The Exceptional Children State forms are available for you to download. Below are instructions on accessing these forms.

  1. Make sure you have version 4.0 or greater of the Adobe Acrobat Reader software installed on your machine. This software is free. For instructions or more information, please visit our PDF download page.

  2. Once you have installed the Reader, open the form you need by clicking on the form name from the list of forms. If the Acrobat Reader is properly installed, this will automatically open the form in Acrobat Reader as either a plug-in (you will see the form within your browser window) or it will launch as a separate window.

  3. You can now print the page by clicking the printer button on the Acrobat Reader menu bar or using the browser's Print feature.
